Well, the patient received a biopsy of the brain lesion (I had offered bronch first but the primary ID team insisted on the neurosurgical approach). The result? Adenocarcinoma.

So my DDX wasn't bad... if I can quote from my previous comment on this post: "Metastatic carcinoma with meningeal carcinomatosis could also look like this and produce a similar LP finding. "
