The patient has some fluid-filled cystic spaces in the retro-sternal and retro-cardiac areas with the interstitial changes outlined by Jennings.
His CT shows large cystic bronchiectasis due to recurrent infections (no TB, no MAC) and he has frequent AECBs with lots of sputum production...
