Follow-up to dyspnea

This is the recent post of the 47 y/o woman with dyspnea. JJ and Krayem suggested HP and on the TBBx we had loose granulomata with lots of lymphocytes and negative organisms on stains and Cxs. The pathologist felt quite confident calling it as HP. Unfortunately I don't know what is the culprit exposure yet...

It is amazing how many cases of HP we've seen where a specific antigen is NOT identified. Either it's difficult to obtain that kind of history, or there are countless HP-causing agents out there that we are not aware of.

Where is she from? i know HP is endemic around the ohio and mississipi and st laurent river in canada. But it looks like there is so many people with Hp or healed granulomas. Is it likely that non caseating hp granulomas cause symtoms? even after the infection has healed?
